Reymond Rodriguez left Little Flower Children's Services in Wading River in an Uber at about 2:30 p.m. on Wednesday, May 11, according to the Riverhead Town Police Department.
Police said no foul play is suspected.
Reymond is described as being 5-foot-7 and 150 pounds, with brown eyes and long brown hair, authorities said.
He was last seen wearing a white t-shirt, blue jeans, and Nike sneakers, police said.
Authorities asked anyone with information about Reymond's whereabouts to call the Riverhead Town Police Department at 631-727-4500 Ext. 312.
